General Medical History

DOT Physical or Medical exam is required by FMCSA for the new and renew commercial driver license. 

Doctor who performed this exam must be certified by FMCSA National Registry.  


First driver will fill out the form MCSA-5875 and answer general medical history. Then examining doctor will go over the medical history with the driver.

Examining doctor or assistant will check the following:   

Height, weight, blood pressure, and pulse.

Blood pressure is a very important part of the exam. If you are taking  medication to control your blood pressure please let examining doctor know that you are taking the medication. You should take your medication when you are going to DOT exam. If your blood pressure reads too high at the time of examination, doctor can disqualify you until your blood pressure is under control according to the FMCSA guide line.

Do you wear eye glasses or contact lens? Make sure to wear and bring them with you on the day of DOT exam.

Vision exam is important part of DOT exam.

Standard is at least 20/40 acuity test.  Which means a person with 20/40 vision sees things at 20 feet that most people who don’t need vision correction can see at 40 feet. At least 70 degree field of horizontal vision. You should be able to distinguish signal light colors.

Standard: Must be able to hear perceive whispered voice at minimum 5 feet. If you are using hearing aid make sure to wear them at the time of examination.  For more information about hearing test.

Urine Test

Urinalysis includes: 

Sugar (glucose), blood, protein, and specific gravity. Main reasons for the urine test is to check for diabetes, kidney diseases and urinary infection.
